What sort of issues were you having with the Signature Application itself? Rather than submitting the details of your signature it would be best to help us understand why you cannot use the form provided - as it would place the thread in the correct forum for us. While you will receive help regarding technical issues on the site there is nobody who can create it for you in this particular area of the forums.
